Case Manager

Position Summary:

This position provides specific program services such as referrals for employment, housing, medical services, and other supportive services. Case manager must provide quality consumer services and case management services in accordance with the standards, values , ethics, and mission of the Agency and abide by the established program policies and procedures.

Essential Duties & Responsibilities

Duties that occupy a major portion of time and importance in the job
  • Meet with consumer at east once a week to provide ongoing services for a specific caseload
  • Complete consumer assessment when assigned new cases
  • Develop consumer Individualized Service Plan with short- and long-term goals
  • Maintain progress notes and secure required documents from consumers and service providers
  • Coordinate referral of consumer to other community agencies as appropriate
  • Complete transitional Housing and Permanent Housing Applications
  • Maintain accurate records and complete necessary statistical reports as required by the Department of Social Services, program contracts or the Agency
  • Escort consumer to housing appointments and provide advocacy Establish community relations including acting as liaison and attending meetings
  • Enter consumer data into the Homeless Management Information System Database (HMIS) daily
  • Assist with Resources Development by way of research and networking
  • Keep supervisor informed of consumer and case matters that require resolution
  • Participate in all required agency meetings and trainings
  • Able to work an o-call rotation flexible schedule
  • Perform other duties as assigned

Qualifications, Knowledge Skill Required

Minimum education and experience needed to perform the job adequately
  • Bachelor’s degree in social work, Human Services, or related discipline. A combination of education, training, and work experience in lieu of degree requirement will be considered.
  • Excellent written and verbal communication skills
  • PC competency and proficiency in MS Office products preferred
  • Effective interpersonal skills
  • Ability to use good problem-solving techniques
  • Attention to detail; record keeping skills; meet deadlines and work well under pressure
  • Demonstrated cultural competence and cultural responsiveness

This work is primarily sedentary, and may require some lifting (up to 15lbs) and include occasional travel to off-site meeting/sponsor locations
